Academic Concerns Description Procedure
Change Section/ Matriculation

This form is needed if the following occurs:

  • His/Her section was dissolved so s/he would need to cancel the subject or move to a different section
  • A new section was opened for a subject s/he can enroll in, so s/he would need to add matriculation; or
  • His/Her section had changed schedules so s/he would need to either change sections for that subject or completely cancel the subject.
  1. Get the Change Matriculation form from the Information Booth during regular registration or at the EEE Administration Office (Rm. 201) during late registration.
  2. Fill out form.
  3. Have it signed by the instructor/s involved and by the adviser (if necessary).
  4. Have it signed by the dean at the Engineering Administration Office.
  5. Pay processing fee at the office of the university registrar (OUR) or any payment center.
  6. Claim the student’s and instructor’s copy. Give the instructor his/her copy.

Leave of Absence (LOA)

A new policy of the Office of the University Registrar (OUR) states that a student must finish his/her LOA during registration if s/he will not enroll at all during the semester. The deadline set in the academic calendar for filing of LOA is only for students who enrolled at the start of the semester but has decided not to finish the semester. 
  1. Fill out the form and attach a letter stating the reason for filing.
  2. Have it signed by the program adviser and the department chairman.
  3. Submit your form to the Engineering Administration Office.

Request for True Copy of Grades/ Filing for Graduation

A student may request for a true copy of grades (TCG) for a specific semester or year, or for the whole duration of his/her stay in the college. This can be used for verification of passing grades in prerequisites, application for scholarships, or student assistantships, etc. Meanwhile, a student must inform the college that s/he is graduating in the next semester or on summer by filing a specific form before a date scheduled by the college.
  1. Get the appropriate form from the Engineering Administration Office.
  2. Fill out form.
  3. Pay the processing fee at the Engineering Administration Office.
  4. For the TCG, inquire with the administrative staff as to when it can be claimed.

Dropping/ Removals/ Completion

DROPPING: A student may decide to drop if he is failing in the subject or if he has exceeded the prescribed amount of absences that a student can have in a semester.

REMOVALS: A student who has incurred a grade of 4.0 may take a removal exam. The removal exam is scheduled by the instructor and may be set anytime during the academic year. Only students who submit a removal form to his/her instructor will be allowed to take the exam.

COMPLETION: A student who has incurred a grade of incomplete (INC) and has submitted the requirement/s s/he needs to complete the subject must fill up a completion form so that his/her final grade will be submitted to the college and to the OUR. S/He must complete the subject either before a deadline set by the instructor or before one (1) year has passed, whichever date is earlier.

  1. Get the appropriate form from the EEE Rm 220.
  2. Fill out the form.
  3. For the completion form, submit it to the instructor. It will now be up to the instructor to submit the final grade to the college and the OUR. Inquire with the EEE administrative staff as to when the student's copy of the form can be claimed.
  4. For the dropping and removal forms, have it signed by the instructor involved, the adviser and the dean.
  5. Pay processing fee at the office of the university registrar (OUR) or any payment center.
  6. Claim the student’s and instructor’s copy. Give the instructor his/her copy.
